All sports physicals will be completed on the IHSA Illinois Pre-participation Form. The student/parent section must be completed prior to the exam. If another form is requested to be completed, that's ok! You'll have the chance to upload during the Booking Request!

Once a date is confirmed, an online consent portal link will be sent for each student's parent to complete!
